If you are carrying out an activity or project please follow the stages as listed below. Any additional forms you may need as well as guidelines and procedures are also here. If you have any questions at any time or you don't think the information you need is listed please don't hesitate to contact a member of staff.
Performance
Step 1 - Complete the Risk Assessment/Project Information form (DOC 26KB)
Step 2 - Complete the Risk Assessment/Onsite Checklist form (DOC 17.6KB)
Step 3 - Complete the Risk Assessment/Evaluation form (DOC 28.3KB)
Step 4 - Meet your Academic Tutor for discussion and project agreement/sign-off
Step 5a - If working off campus contact the relevant local authority or if in York complete the York City Centre Booking Form CCM1 (DOC 0.7 MB)
Step 5b - If working on campus ensure the venue is booked either by your Academic Tutor or via Timetable.

Exhibition

Step 5b - For on-campus projects book the space required via Timetable.

Study Related Driving procedure
Licence Bureau Information
If you need to drive as part of your course, ie travel to a location for filming or an exhibition etc you must complete sections 2 and 4 of the Licence Bureau form. Once completed you should send it directly to the Bureau, either via email at consent@licencebureau.co.uk or by post, the address for which is on the form.
Anyone who has not had their licence checked via this method and is therefore not on the Bureau’s database will not be permitted to drive and may be stopped from doing so; you may also be unable to claim back your expenses for the mileage.
